duḥkham api adhikaṁ citte — even intense pain, in the heart; sukhatvena eva rajyate — is colored precisely as pleasure, i.e. relished as joy.
Even intense suffering, once it is in the heart, gets relished exactly as if it were pure pleasure.
This is the lakṣaṇa (defining mark) of rāga. Rūpa defines it here: when love has risen so high that even great pain is felt as joy, that state is rāga — a stage above praṇaya, as L2 states.

Jīva supplies a missing condition to the verse. This turning of pain into joy happens even when getting Kṛṣṇa is only a prospect, not yet a fact — the mere hope of Him is enough.

Viśvanātha refines the words. 'Pain' here means the cause of pain; 'as pleasure' means it shows itself as a cause of pleasure. So the very thing that would normally cause pain now works as a cause of joy — and this, whenever getting Kṛṣṇa is felt to be within reach.
Viśvanātha reads duḥkha not as the feeling of pain itself but as its cause (duḥkha-kāraṇa), and sukhatva as being a cause of pleasure — sharpening the mūla so it speaks of the vibhāva (the cause) that is re-tasted, not merely the raw sensation.

He unpacks 'as pleasure' as 'through the pleasure-quality': in such a case of pain, only the pleasure-quality is felt, never the pain-quality. The picture: wax dyed with cinnabar — you register only the cinnabar's red, not the wax beneath. Map it across — the wax is the pain; the cinnabar-red is the pleasure-quality lent by heightened praṇaya; what you actually feel is only the red, only pleasure, never the wax, never the pain. The analogy proves the pain is genuinely experienced as joy; it stops there — it does not say the pain has vanished, only that it goes unfelt beneath the pleasure-color.

A second picture makes the same point: a cloth dyed with madder shows only the madder's red, not the cloth's own whiteness. Map it — the white cloth is the pain, the madder-red is the pleasure-quality, and you feel only the red (the pleasure); the cloth's native whiteness (the pain) goes unfelt.
