The verse opens describing the waves that seize her: they throw her limbs about and come with thick, loud, raving babble — and all of it in full flood.
वे भीतरी तरंगें उसके अंगों को झटके देकर फेंकती हैं। उनसे वह गाढ़े और ऊँचे स्वर में प्रलाप करने लगती है। यह सब भरपूर मात्रा में होता है।
An udāharaṇa (illustrative verse, in a sakhī's voice) of apasmāra, 'loss of consciousness / epileptic collapse', a vyabhicāri-bhāva of prakaraṇa 13. The lead-in reads 'atha apasmāraḥ, yathā—' ('now apasmāra, as follows'). Its lakṣaṇa is the one Jīva and Viśvanātha give below: a derangement of mind arisen from humoral imbalance brought on by pain.
यह अपस्मार (मूर्च्छा / मिर्गी जैसा गिर पड़ना) का उदाहरण-पद्य है, जो एक सखी के मुख से कहा गया है। अपस्मार प्रकरण १३ का एक व्यभिचारी-भाव है। पद्य से पहले की पंक्ति है 'अथ अपस्मारः, यथा—' ('अब अपस्मार, जैसे कि—')। इसका लक्षण वही है जो आगे जीव और विश्वनाथ देते हैं — दुःख से धातुओं के बिगड़ जाने पर उत्पन्न मन की विकलता।

Two more marks the waves leave on her: her pupils are rolled far up in their sockets, and she throws up sprays of froth. These are the classic outward signs of a fit.
उन तरंगों से उसकी पुतलियाँ ज़ोर से ऊपर की ओर उलट जाती हैं। और मुख से झाग की धार बहने लगती है।

Now the address and the real cause: O Kṛṣṇa, it is waves of inner turmoil that seize my friend — and those waves rose up from her separation from you. Everything described so far is the effect of viraha.
हे कृष्ण! तुम्हारे विरह से मेरी सखी के भीतर विकार की तरंगें उठ आई हैं। इन्हीं तरंगों ने ऊपर कही सारी दशा उत्पन्न की है।

Here is the payoff. The elders watch my friend gripped by all this and now conclude she has apasmāra — epilepsy. The 'now' quietly says she was never like this before. The whole point of the example: her viraha-symptoms look exactly like a disease, so they misread love as illness.
इन तरंगों से ग्रस्त मेरी सखी को देखकर घर के गुरुजन अब यही अनुमान लगाते हैं। वे समझते हैं कि इसे अपस्मार (मिर्गी) का रोग हो गया है।

Jīva first defines the disease. Apasmāra is a derangement of the mind. It comes from an imbalance of the bodily humors — and things of that sort — which suffering sets off. So the ordinary illness has a physical, pain-driven cause.
अपस्मार चित्त का विप्लव है, अर्थात् मन की विकलता। यह दुःख से धातुओं के बिगड़ जाने आदि से उत्पन्न होता है।
dhātu here = the bodily humors/constituents of Āyurvedic medicine; their vaiṣamya (imbalance) is the standard physiological cause of the disease apasmāra.
यहाँ 'धातु' का अर्थ है आयुर्वेद में बताए गए शरीर के दोष / मूल तत्त्व। उनका वैषम्य (असन्तुलन) ही अपस्मार रोग का सामान्य शारीरिक कारण माना जाता है।

Now Jīva pins down the verse-word. 'Apasmāriṇīm' means: a woman who has the apasmāra people commonly know — that is, the ordinary disease, exactly what the elders think.
'अपस्मारिणी' का अर्थ है — लोक में जो साधारण अपस्मार रोग प्रसिद्ध है, उससे युक्त स्त्री।

Jīva closes the contrast. The word 'apasmāriṇīm' fits only the common disease. It does not mean she is stricken by what is really born of her great love for you — emphatically not that. The elders' label catches the symptom but misses the true cause.
पर यह अर्थ उस दशा का नहीं है जो तुम्हारे प्रति महान अनुराग से जन्मी है। निश्चय ही वह अनुराग-जन्य अवस्था यहाँ अभिप्रेत नहीं है।
The sentence carries a doubled negation (na tu ... na); read as emphatic — 'certainly not the love-born condition'.
इस वाक्य में नकार दो बार आया है (na tu ... na)। इसे बल देने के लिए समझना चाहिए — 'निश्चय ही वह अनुराग-जन्य दशा नहीं'।

Viśvanātha sets the scene. Śrī Kṛṣṇa has gone to Mathurā, so this is spoken from separation. Lalitā, through some messenger, sends him this message — and the message is the verse beginning 'aṅga...'. So Viśvanātha names the sakhī as Lalitā.

Viśvanātha now fills in the verse's referents. The waves of inner turmoil that grip her rose from her separation from Kṛṣṇa. And the 'elders' who misdiagnose her are her in-laws — father-in-law, mother-in-law and the rest — who take her for a woman with the ordinary disease apasmāra.
तुम्हारे विरह से मेरी सखी के भीतर विकार की तरंगें उठी हैं और उससे वह ग्रस्त है। श्वसुर, सास आदि गुरुजन उसे लोक-प्रसिद्ध अपस्मार रोग वाली मान बैठते हैं।
By naming the guravaḥ as śvaśura and śvaśrū (father- and mother-in-law), Viśvanātha places the scene in the parakīyā (married-gopī) setting: it is the husband's household that mistakes her love-symptoms for disease.
गुरुजन को श्वसुर और सास बताकर विश्वनाथ इस दृश्य को परकीया (विवाहिता गोपी) की स्थिति में रखते हैं — पति के घरवाले ही उसके प्रेम-लक्षणों को रोग समझ बैठते हैं।
