The verse opens on Rādhā's condition: her end is rising up, death coming on, and the very attendants who might cure her have gone slack in mind at even beginning a remedy.
An udāharaṇa of ādhi (anxious mental distress), a vyabhicāri-bhāva, quoted from Haṁsadūta 95; in it Lalitā, sending word to Kṛṣṇa through the swan, describes Rādhā's near-death state so that he will come at once.

ata eva — just because of this; parijanaiḥ asmad-vidhaiḥ — by attendants such as us; iyaṁ na jīviṣyati iti niścayena — sure she will not survive; tat-kadanasya pratīkārārambhe ślatha-matibhiḥ — slack of mind at beginning a cure for that agony; vimuktāyāḥ — abandoned.
Because her death is rising, the attendants — 'people like us,' Lalitā says — have concluded she cannot live. So their will to begin any treatment for her agony has gone slack, and they have left her. That is why she is 'abandoned.'

'Abandoned' has a concrete sense: she has been laid on the bare ground, on a deathbed of earth.

Now a fresh image. Hope is like a physician who, even faced with an incurable disease, sets out to treat the patient with her own power — and will not walk away. That is why the verse says she 'does not let go her embrace': hope stays at the bedside and keeps working. The mapping: hope is the doctor; Rādhā's fatal love-sickness is the incurable disease; refusing to release the embrace is the doctor who never leaves. The analogy shows hope's devotion; it does not claim hope can actually cure death — only Kṛṣṇa's coming can.

tena tat-preyasīṁ sā āśā sahacarī eva jīvayantī samprati vartate — that hope, as companion, is now keeping your beloved alive; sā ca avadhi-divasāt para-dine na sthāsyati — but it will not hold beyond the set day; iti bhavatā na jñāyate eva — which you do not know; iti etan-madhye śīghram āgantavyam iti dhvaniḥ — so the hint is: come quickly within this time.
Viśvanātha draws out the suggestion. Right now this hope, like a faithful companion, is keeping Kṛṣṇa's beloved alive. But the hope will not survive past the promised day — and Kṛṣṇa does not even realize this. So the real message is: come, and come quickly, before the deadline passes.
