An udāharaṇa of a sāttvika-bhāva arising from vismaya ('wonder'), announced by the lead-in vismayād yathā (12.15). Two of the eight sāttvikas surface in the verse: Rādhā's own throat stunned into silence (stambha), and the horripilation (pulaka) that breaks out even on the creepers at Kṛṣṇa's flute. Per Viśvanātha this is Rādhā speaking to Lalitā.

You did not even notice the goosebumps standing up on the creepers! The wonder is that Kṛṣṇa's flute thrills even the vines into horripilation, and Rādhā — herself too choked to speak — tries to point it out by hand, but Lalitā still misses it.

tena stimitaḥ stabdhaḥby that rush the throat is stimita — that is, stabdha, frozen stiffउस (आवेग) से स्तिमित अर्थात् स्तब्ध
kaṇṭhaḥ yasyāḥ tayāshe whose throat is so — 'by her', i.e. by me [Rādhā] resolves the verse's compound -kaṇṭhayā mayā as a bahuvrīhiजिसका कण्ठ (स्तब्ध हो गया) उस (राधा) के द्वारा
śrī-rādhā lalitām āha — Rādhā says to Lalitā: guru-sambhramaḥ — the intense agitation is abhisāra-artham āvegaḥ — the rush for the tryst; tena stimitaḥ stabdhaḥ kaṇṭhaḥ yasyāḥ tayā — she by whom [i.e. by me] the throat is frozen stiff by that rush.
Viśvanātha sets the scene: this is Rādhā speaking to Lalitā. The 'intense agitation' of the verse is the rush she feels to reach the tryst (abhisāra). That rush froze her throat stiff — so the verse's phrase means 'by me, whose throat was stopped this way.'
abhisāra: a nāyikā's going out to meet her beloved at the trysting-place — a standard situation of Sanskrit love-poetics; here it is the wonder-mixed haste that chokes Rādhā's voice.

So, since her throat was stopped and she could not speak plainly, she resorted even to hand-signs — moving her hand — to get the message across. This unpacks the verse's 'even by hand-signals.'

saṁjñā syāt — 'saṁjñā' may mean cetanā nāma — consciousness, ca — and hastādyaiḥ artha-sūcanā — the signalling of meaning by the hands etc.; iti amaraḥ — thus the Amara-koṣa.
Viśvanātha backs the reading with the dictionary: the Amara-koṣa lists two senses of saṁjñā — 'consciousness', and 'conveying a meaning by the hands and so on.' The second sense is the one at work in the verse.
Why cited: Lexical proof from the Amara-koṣa that saṁjñā denotes signalling meaning by hand-gestures, grounding the verse's kara-saṁjñā as hand-signalling rather than mere 'consciousness'.
