That is Kṛṣṇa there — look. His fingers are clamped tight against the tip of his thumb (that is the milking grip), and he holds the ground down under half his foot, crouched low to milk.

O friend — Dāmodara is milking the cow! The milk is lovely with the very sound of its streams pattering down. He milks the cow and he milks the milk — the one verb carries both.
हे सखी! दामोदर गाय को दुह रहा है। दूध की धाराओं की ध्वनि से वह दृश्य मनोहर हो उठता है।
duh, 'to milk', is a classic dvi-karmaka (two-object) verb: it takes both the source milked (gām, the cow) and the thing drawn out (payaḥ, the milk) as direct objects. Viśvanātha flags exactly this in his gloss (s6): payo gām iti karma-dvayam.

First Viśvanātha tells us who is speaking: it is Viśākhā, pointing Kṛṣṇa out to Rādhā, who says all this. Then he unpacks the opening compound. 'Agrima' of the thumb means its foremost part — the tip. So aṅguṣṭhāgrima-yantritāṅguliḥ describes Kṛṣṇa as the one whose fingers are held clamped tight by that thumb-tip.

A small grammatical point: aṅguṣṭhāgrima literally reads 'thumb-front', but it means 'the front part of the thumb'. So the word 'bhāga' (part) has to be taken as absorbed silently inside the compound — you supply it in sense even though it is not written.

Its example: 'an ornament that is a modification (vikāra) of gold' collapses into suvarṇālaṅkāra, 'gold-ornament' — where 'modification' has dropped away but is still understood. Just so, 'part' is understood inside aṅguṣṭhāgrima.

Now the long lead-in phrase. His two knees bend a little low, and between them the milk-pot is clamped, set down and held still. Into the mouth of that pot the streams slip and fall — and their very sound makes the scene charming. That is what dhārādhvāna-manoharam catches. Last, he notes the grammar: 'milk' (payaḥ) and 'cow' (gām) are both objects of the one verb 'milks' (dogdhi).
