This verse is an udāharaṇa of asūyā-garbhā īrṣyā, 'jealousy laced with fault-finding,' which Viśvanātha (s1) sets against the śuddha-īrṣyā, 'pure jealousy,' of the previous verse. The speaker is Padmā (so Viśvanātha), who meets a friend now wearing the necklace and, out of envy, runs it down — pretending it was hers to refuse.

prājyaṁ doṣam avekṣya nāyaka-maṇau na svīkṛto’bhūn mayā |
prājyaṁ doṣama great flaw prājya = 'abundant, plentiful' — so both Jīva and Viśvanātha gloss itप्रचुर (बहुत बड़ा) दोष को
avekṣyaonce I had noticed itदेखकर
nāyaka-maṇauin the central gem nāyaka-maṇi = the chief jewel at the necklace's middle (Viśvanātha: hāra-madhya-nāyaka); but nāyaka also = 'the hero, the lover,' so the fault is being aimed, under cover, at the beloved himselfनायक-मणि में (हार के बीच की प्रधान मणि में — और छिपे अर्थ में 'नायक' अर्थात् प्रियतम में भी)
na svīkṛtaḥ abhūtwas not acceptedस्वीकार नहीं किया गया
mayāby meमेरे द्वारा
prājyaṁ doṣam avekṣya — noticing a great flaw; nāyaka-maṇau — in the central gem; mayā — by me; na svīkṛtaḥ abhūt — it went unaccepted.
…I would not take it — I saw a great flaw in its central gem. So I, of all people, refused it.
The śleṣa turns on nāyaka-maṇi. On the surface it is the 'lead gem' at the heart of the necklace; but nāyaka is also 'the hero / the lover.' The flaw Padmā claims to spot 'in the central gem' is thus, at the veiled level, a slur cast at the nāyaka — Kṛṣṇa himself. Faulting what is in fact faultless is exactly the asūyā, the detraction, that fills this species of jealousy.

prājyaṁ — pracuram; i.e. 'abundant.'
Jīva glosses the single hard word: prājya means 'abundant, plentiful' — hence 'a great flaw.'

kutaḥand why not?
nāyaka-maṇau hāra-madhya-nāyakein the nāyaka-gem — that is, the lead jewel set at the necklace's middle Viśvanātha fixes the surface sense: nāyaka-maṇi = the central gem; the 'hero/lover' pun rides underneath'नायक-मणि' अर्थात् हार के बीच की प्रधान मणि में (ऊपरी अर्थ; 'नायक/प्रियतम' का श्लेष नीचे बहता है)
prājyaṁ pracuraṁ doṣaṁbecause it carries a great, abundant flaw again prājya = pracura, echoing Jīva'प्राज्य' अर्थात् प्रचुर दोष को (फिर जीव की ही व्याख्या दोहराते हुए)
dhārayituḥ asaubhāgya-karaṇa-lakṣaṇakama flaw of the sort that brings misfortune on whoever wears it dhārayitṛ = the wearer (genitive); the flaw's mark is asaubhāgya-karaṇa, 'ruining her good fortune'पहनने वाली का दुर्भाग्य करने के लक्षण वाला (दोष)
kutaḥ — why untaken?; nāyaka-maṇau hāra-madhya-nāyake — because in its central gem; prājyaṁ pracuraṁ doṣaṁ — a great, abundant flaw; dhārayituḥ asaubhāgya-karaṇa-lakṣaṇakam — one that spells ruin for the wearer's fortune.
Why refuse it? Because of the flaw in its central gem — the lead jewel at the necklace's heart. A great flaw, she says, the kind that wrecks the good fortune of anyone who wears it. (And 'nāyaka' being also 'the lover,' the charge lands, veiled, on Kṛṣṇa.)

Say Kṛṣṇa himself gave it and you love it — give it up anyway. If parting stings, bear the sting. Then comes the simile that carries the whole point: a finger bitten by a snake is your own flesh, yet you cut it off — because if you don't, the poison spreads and kills you. Map it across: the necklace is the dear finger; its flaw is the snakebite; casting it off is the amputation; and what is saved is the wearer's good fortune, her very life.
