This line paints the ear-scratching itself. As her hand moves, the bangles on her wrist jingle. The sound is so like Kāma's kettledrum that one half-suspects the love-god is beating his war-drum. It is a poetic fancy: the ornaments' tinkle becomes Love's own instrument.
बजते हुए कंगनों से मानो कामदेव के नगाड़े की-सी ध्वनि का भ्रम उत्पन्न हो जाता है। ऐसी उस कान खुजलाने की क्रिया को —

Viśvanātha unpacks the long compound word by word. The 'red finger' is the reddish little finger of the left hand. Rubbing with its tip means she slips the fingertip into her ear-hole and works it. This sets the jingling bangles on her hand swaying. By their sound arises the suspicion — he says plainly, the illusion — of Kāma's kettledrum, a special instrument of the love-god. So the whole word means 'that ear-scratching in which' this illusion is created.
विश्वनाथ इस समास को खोलते हैं। रक्ताङ्गुली से तात्पर्य है — बाएँ हाथ की अरुण वर्ण की कनिष्ठा अँगुली। उसके अग्रभाग को कान के छिद्र में डालकर हिलाने से हाथ में पड़े कंगन चंचल होकर बज उठते हैं। उन्हीं कंगनों की ध्वनि से मानो कामदेव के किसी नगाड़े का भ्रम हो जाता है — यहाँ शंका का अर्थ भ्रम ही है।
