Because this whole speech is just teasing. The girlfriend Padmā made it up on her own — both the 'sadyastana' words (the talk of a fresh scratch) and the line that 'the husband's sister, standing far off, is watching and calling out.' She says it all as pure fiction, not as fact.
चूँकि यह परिहास का वचन है, इसलिए 'सद्यस्तन' आदि बातें सखी ने स्वयं मन में गढ़कर कहीं — कि 'ननद यद्यपि दूर खड़ी थी, फिर भी आवाज़ करती हुई ताक रही है' — यह सब केवल झूठ-मूठ ही कहा गया।
jīva names the onlooker as the husband's sister (patuḥ svasā), where the verse line itself says the cowherd (gopaḥ, grown angry). Left as jīva states it — not harmonized with the mūla or with viśvanātha.

And it has to be fiction, for a simple reason: girls of good family would never actually bare their breasts for an inspection. Since that could not really happen, the whole scene is a joke.
