This first line gives the raw material of the device. Something is seen, or heard about, or lived through directly. Then it is told — spoken out, narrated, described. Out of that telling a resemblance is born: a likeness set up between what is being described and the situation at hand. The whole phrase is one long adjective waiting for its noun — 'the resemblance that springs up from telling of things seen, heard, or undergone' — and that noun, sādṛśyaṁ, opens the next line.
जो अर्थ देखा गया, सुना गया या स्वयं अनुभव किया गया है, उसके कहने आदि से जो सादृश्य उत्पन्न होता है — यह सादृश्य की सामग्री है। देखने, सुनने और अनुभव करने — ये तीनों ही उस बात के स्रोत हैं जिसका कथन होता है।
This kārikā is the lakṣaṇa (defining verse) of sārūpya, the device named in this subsection (sārūpyaṁ). L1 states its material — a resemblance born of telling things seen, heard, or undergone; L2 will add the trigger (agitation) and pronounce the name. (The Prakrit Rādhā verse standing in the lead-in belongs to the previous device, anukta-siddhi, which it closes with 'atrānuktasyāpi… anukta-siddhiḥ' — it is not the illustration of sārūpya.)

Now the trigger and the name. When, out of saṅkṣobha — an inner shaking-up, a rush of feeling — such a resemblance springs up, that case is expounded as sārūpya, 'the matching of forms.' So sārūpya is not just any comparison: it is a likeness that bursts out under emotional turmoil, as a person, stirred up, speaks of things seen, heard, or lived through and thereby likens them to what is before them.

The definition has two hinges: the resemblance must come through telling (L1), and it must be driven by agitation (saṅkṣobhāt, L2). Take away the agitation and you have a plain simile; add it, and the likeness becomes this dramaturgic aṅga. nirūpyate is the standard closing verb of a lakṣaṇa — 'and this is what gets set down under the name sārūpya.'
