sudhāṁśu is 'the one whose rays are nectar', a name for the moon; so 'moon-faced' is said with the word sudhā, 'nectar', inside it, right beside mādhurī, 'sweetness' — the friend's own face is nectar while the flute's nectar is unsettling her.
'सुधांशु' अर्थात् 'जिसकी किरणें अमृत हैं' — चन्द्रमा का नाम। इस प्रकार 'चन्द्रमुखी' कहने में 'सुधा' (अमृत) शब्द भीतर बैठा है, और उसके पास ही 'माधुरी' (मिठास) खड़ी है — सखी का अपना मुख अमृत है, और उधर मुरली का अमृत उसे विचलित कर रहा है।

śrūyamāṇaṁ muralī-ravaṁ lakṣīkṛtya kācid āha—kāmam iti ||
śrūyamāṇaṁ muralī-ravamthe flute-sound that is being heard right then accusative object of lakṣīkṛtya; śrūyamāṇam is a present passive participle — the flute is sounding as she speaksसुनाई पड़ रही मुरली की ध्वनि को — कर्मवाच्य वर्तमान कृदन्त, अर्थात् बोलते समय मुरली बज ही रही है
lakṣīkṛtyataking that as her target, she speaks with it in view gerund: 'having made [it] the mark'उसे लक्ष्य बनाकर

Jīva leaves the speaker as kācit, 'some woman', and does not name her or the friend she addresses.
जीव गोस्वामी बोलने वाली को 'काचित्' अर्थात् 'कोई एक स्त्री' ही रहने देते हैं; न उसका नाम बताते हैं, न उस सखी का जिससे वह कह रही है।

Viśvanātha unpacks the verse this way. O moon-faced one — the sweetness of the flute is setting your heart quivering. That is what I have understood. He fills in the missing agent: it is I who have understood it.
हे चन्द्रमुखी! मुरली की माधुरी तेरे चित्त को चंचल कर देती है — ऐसा मैंने जान लिया।

tatra hetuḥand here is what she is going on — the reason for saying she knows tatra points back to the mayā jñātam just glossed; hetuḥ names it as the ground of that inferenceउसमें हेतु यह है — 'तत्र' अभी-अभी व्याख्यात 'मया ज्ञातम्' की ओर संकेत करता है, और 'हेतुः' उसे उस जानने का कारण बताता है
