muralī-nisvāna-śuśrūṣābecause she longs to hear the sound of the flute śuśrūṣā is 'the wish to hear'; Viśvanātha construes it as the cause of the listening (muralī-dhvani-śuśrūṣayā, instrumental)जिसे मुरली की ध्वनि सुनने की चाह है — 'शुश्रूषा' अर्थात् सुनने की इच्छा; विश्वनाथ इसी इच्छा को सुनने का कारण मानकर तृतीया में पढ़ते हैं
adya — today; muralī-nisvāna-śuśrūṣā — because she longs to hear the flute's sound; vinyasta-śruti-pāliḥ — she has set the tips of her ears to listen
Today she is listening for the flute. Her ears are turned toward it, straining for that one sound.
An udāharaṇa of vātsalya-rati, the sthāyī-bhāva of this rasa, defined at 3.4.52 — the rati of one who feels tenderness toward the object of that tenderness, with awe and the rest fallen away. The lead-in yathā vā marks it as a second example after the Bhāgavatam verse at 3.4.54.

Jīva reaches for the dictionary. Viśva-prakāśa says pāli means the tip of the ear-lobe.
Why cited: a lexicon citation fixing the sense of pāli in śruti-pāli as the ear's tip, not the whole ear — which is what the next sentence then has to account for.

samagra-karṇa-vinyāsaḥ evathe setting of the whole ear is what is meantपूरे कान का लगाना ही
lakṣyateis what the word points to the technical verb of lakṣaṇā, indication: the literal word says 'tip', the intended sense is 'ear'लक्षित होता है — यह लक्षणा का पारिभाषिक क्रियापद है: शब्द कहता है 'सिरा', पर अभिप्रेत अर्थ है 'पूरा कान'
tad-vinyāsena tu — but by the setting of that tip; samagra-karṇa-vinyāsaḥ eva — the setting of the whole ear precisely; lakṣyate — is what is indicated
Strictly, pāli is only the tip. But when the tip is turned toward a sound, the whole ear is turned. So the word points past its literal sense: she has cocked her whole ear.
lakṣaṇā is the standard move in Indian poetics where a word's literal sense is kept but stretched to what it points at — like saying 'the village on the Gaṅgā' when you mean the bank, not the water.

Viśvanātha turns the word agra, 'tip', a second way. Other talk may reach her ear; she hears it. But the ear's foremost work — the top of what it is doing — is fixed on the flute alone. That is the sense.
Two readings of agra-bhāga stand together here: the physical tip of the ear (from the dictionary) and the ear's highest function (Viśvanātha's turn). The dictionary sense licenses the poetic one; both are needed for the sentence to land.

Raining streams means streams of milk. Her breasts are flowing.

prasrava on its own is any flow; Viśvanātha fixes it to breast-milk, which is what makes the line a sign of vātsalya-rati rather than a weather image.
