atra kiñcit kṛśaṁ madhyam īṣat-prathima-bhāg uraḥ |
atrahere — that is, in this last stretch of kaumāra now being taken up the lead-in is atha śeṣam, 'now what is left'; atra points to that third and final stretch, after the first and middle have been describedयहाँ, अर्थात् अब जो शेष बचा है उस तीसरे और अन्तिम खण्ड में

atra — here, in the last kaumāra; madhyam — the waist; kiñcit kṛśam — is somewhat slender; uraḥ — the chest; īṣat-prathima-bhāk — takes on a slight breadth
Now the last stretch of kaumāra. The waist, which was plump before, has thinned a little. The chest has started to broaden, but only slightly.
Read against 3.4.18-19, where the first kaumāra had a plump waist. The body-marks are the tally by which the age-stage is recognised, and each stage is drawn by contrast with the one before.

saḥ — he; manāk apacīyamāna-madhyaḥ — with his waist slightly thinning
Now the illustration. He — the boy in his last kaumāra — his waist is quietly slimming down.
An udāharaṇa of carama-kaumāra, the last stretch of boyhood, defined just above in 3.4.29 ('the waist somewhat slender, the chest slightly broad, the head rich with side-locks'). The verse walks the same three marks in the same order, then adds the effect on the mother.

And so the heavenly child left his mother stock-still. She just stood there, staring. The three body-marks are the uddīpana; the mother's stunned stillness is the effect in her.

stambha, 'being frozen stiff', is one of the eight sāttvika-bhāvas — the involuntary bodily signs of a risen mood. Viśvanātha reads the causative exactly this way: the boy made her yuktā, joined to the stambha-bhāva.

apacīyamānetithe word apacīyamāna in the verse — he quotes it to explain its form apacīyamāna + iti, the standard way a commentator picks up a word for comment'अपचीयमान' — यह पद लेकर (अपचीयमान + इति; टीकाकार इसी ढंग से व्याख्या के लिए शब्द उठाता है)
karma-kartari prayogaḥis used in the karma-kartṛ construction — the thing acted on is spoken of as if it were the doerयहाँ कर्म-कर्तरि प्रयोग है
apacīyamāna iti — as for the word apacīyamāna; karma-kartari prayogaḥ — it is a usage in the object-as-agent form
Jīva stops at one word: apacīyamāna. It looks passive — 'being reduced'. But it is the karma-kartṛ usage, where what is acted on is put as the doer. So the waist is not being thinned by anyone; it thins.
karma-kartari is a term from vyākaraṇa (Pāṇinian grammar). At home it names sentences like odanaḥ pacyate, 'the rice cooks (itself)' — the object takes the agent's slot when the action needs no outside doer. Here it keeps the verse from implying that someone or something is slimming the boy's waist; it slims on its own, with growth.

So the plain sense: his waist is getting thin by itself. Jīva rewrites the passive participle with an active one, bhavat, to make the point stick.

Then Jīva defines the hairstyle. Three braids: one over the left, one over the right, one down the middle. Where the three meet at the back of the head — that meeting is what the verse calls kāka-pakṣa. The name means 'crow's wings', from how the locks spread.
Jīva gives only this one definition and lets it stand. Viśvanātha gives the same one, then adds a second opinion held by others; do not read the two commentators as agreeing on a single settled answer.

Now the second body-mark. Viśvanātha swaps the poetic prathiman for the everyday vistāra, 'widening', and then unties the compound: the boy is the one whose chest is asking to learn how to start broadening. The yasya-saḥ formula tells you it describes him, not something separate.

Last line. The divine boy made his mother stambha-bhāva-yuktā — joined to the state of being frozen.
