mathurāyāmin Mathurāमथुरा में
iha — here; madhure mathurāyām — in sweet Mathurā; dīvyati — he is playing
Here in sweet Mathurā he is enjoying himself.
The lead-in reads mūrcchitam— , 'fainting'. So this verse is the illustration of that state, spoken in a character's voice; the whole verse is one friend's message sent to Kṛṣṇa.

madhu-nāthe — now that the lord of Madhu; adhunā rājyam prāpya — having got the kingdom at last; [dīvyati] — is playing
The lord of Madhu has won his kingdom now. And so he plays.
madhu-nātha is a śleṣa: 'lord of the Madhus' (the dynasty, the city) and 'lord of sweetness'. The verse keeps sounding madhu against madhura — the sweetness is all on Mathurā's side of the picture.

ruditāndha can be heard two ways at once: eyes swollen shut from crying, and eyes that see nothing because the one they looked at is gone.

Jīva sets the scene. The verse is not a narrator's report. It is a message, sent to Kṛṣṇa by one particular friend.
viśeṣa here means 'a specific one, not just anybody' — Jīva marks the speaker as a definite person without naming who.

The verse never says the word 'fainting'. Jīva says it is suggested — by ruditāndhe and the words around it. And not one faint: again and again.
dhvani, in Sanskrit poetics, is the sense a verse makes you hear without saying it. That is exactly the point here: the lead-in promised mūrcchā, and the verse delivers it by suggestion.

Why does 'blind' mean fainting? Because of what follows crying. You cry, and then you black out — again, and again. That blackout is the blindness the verse names.
