Unpack the compound this way. Separation is not merely like a fever; separation is the fever. Saṁjvara then is what that fever throws off — plain burning heat. So the line reads: burnt by the heat of the fever that is separation from you.
विरह ही ज्वर है। उसी ज्वर के संज्वर से, अर्थात् संताप से।
Two words from the same root sit side by side: jvara, the fever, and saṁjvara, its flaring. The gloss santāpa keeps saṁjvara from being read as a second, separate fever.
