An udāharaṇa of klama, 'weariness / exhaustion' — the vyabhicāri-bhāva introduced by the lead-in klamena, yathā— in the caturtha-laharī (2.4). The klama is shown by nitānta-tāntā and by her dropping off to sleep after union in L2.

suratasya sambhogasya ante — at the end of union; śrama-jalena — by the sweat; saṅkrāntaṁ mīlitaṁ — transferred, i.e. smudged together; gairikādi-kṛta-makarikādi-citraṁ — the makarikā and other designs made with red ochre and the like; yasyāḥ — of whom; sā viśākhā — she is Viśākhā.
Viśvanātha unpacks the compound saṅkrānta-dhātu-citrā. At the end of the union, the sweat of exertion made her painted designs — the makarikā and other patterns done in red ochre and such pigments — run and smudge together. She whose designs are thus blurred is Viśākhā.
makarikā: a decorative pattern (often drawn on the body with fragrant pigment). Viśvanātha reads the mūla's dhātu ('mineral') as gairika red ochre and citra as these painted patterns, with saṅkrānta glossed by the synonym mīlita, 'blurred together'.
