yat yasmāt — because; ayam unmādaḥ — this unmāda; tatra vyādhiṣu madhye — in the midst of the vyādhis; viccheda-janya-vipralambha-rase — in the separation-born vipralambha-rasa; mohanatvaṁ prāpte adhirūḍha-saṁjñake mahā-bhāve — in the adhirūḍha-named mahā-bhāva that has reached mohana; parāṁ sarvotkṛṣṭāṁ vaicitrīṁ vailakṣaṇyaṁ kurute — produces the highest wondrous variety, its distinctiveness; ataḥ — therefore; asau — this one; vyāpibhyaḥ sakāśāt avasthāntaraṁ prāptaḥ — having reached a state apart from the pervading moods; iti hetoḥ — for that reason; divyonmāda ucyate — is called divine madness.
Viśvanātha spells out the whole argument. He reads the verse's 'yat' as 'because', and 'there' as 'among the vyādhis'. In the vipralambha-rasa born of actual separation, once the adhirūḍha-mahā-bhāva has reached mohana, this unmāda works up the very highest wondrous variety — its sheer distinctiveness, seen in the anubhāvas and sāttvika-bhāvas. Because it has thereby crossed into a state apart from the ordinary moods that pervade every rasa (the vyāpins), it gets its own name: divya-unmāda.
'vyāpin' here = the vyabhicāri-bhāvas that pervade all rasas generally; Viśvanātha's point is that divya-unmāda has stepped out of that pervasive class into a state of its own — which is what 'avasthāntaram' in line 4 means.
