sahasācoming on all at once, suddenly adverb — the key mark: it strikes without warningअकस्मात्, बिना किसी पूर्वसूचना के
manaḥ-kampaḥ — a trembling of the mind; gātrotkampī — [that] makes the limbs shake; sahasā — suddenly; trāsa ucyate — is called trāsa.
A trembling of the mind that shakes the body and comes on suddenly is called trāsa (alarm). The two marks are: it shakes the limbs, and it strikes all at once.
The lakṣaṇa of trāsa, one of the thirty-three vyabhicāri-bhāvas treated in this caturtha-laharī (Dakṣiṇa-vibhāga 2.4).

pūrvāpara-vicārotthaṁ bhayaṁ trāsāt pṛthag bhavet ||
pūrvāpara-vicārotthaṁrising from weighing what came before and after compound qualifying bhayaṁ — this is the mark that sets bhaya apartआगे-पीछे के विचार से उठने वाला
bhayaṁfear (bhaya)भय
trāsātfrom trāsa ablative of separation — kept distinct fromत्रास से (पञ्चमी — उससे अलग)
pṛthag bhavetshould stand as separateपृथक् होता है
bhayaṁ — fear; pūrvāpara-vicārotthaṁ — rising from weighing before-and-after; trāsāt — from trāsa; pṛthag bhavet — stands separate.
Fear (bhaya), by contrast, rises from thinking over what came before and what may come after. On that account it is a separate thing from trāsa — trāsa is sudden and thoughtless, bhaya is reasoned.

To separate the trāsa named earlier from bhaya, Rūpa speaks the verse beginning 'gātra…'. So Jīva reads the whole verse as one job: keeping the two apart.

atra — here; manaḥ-kampaḥ — 'trembling of mind'; pūrvokto hṛt-kṣobha eva ucyate — means precisely the earlier-named agitation of the heart.
The verse's 'trembling of mind' is nothing but the hṛt-kṣobha, the shaking of the heart, spoken of earlier. Jīva pins the term down so it is not taken as some new state.

And 'sahasā' means: without any weighing of before and after. Jīva makes 'suddenly' carry the whole contrast — sudden here means thoughtless, unreasoned.

atarkite tu sahasā — 'sahasā = in the unexpected'; ity amaraḥ — so the Amara-kośa.
He backs this with the dictionary: the Amara-kośa gives 'sahasā' the sense of 'in the unexpected, the unanticipated'. So the lexicon itself supports reading 'suddenly' as 'without forethought'.
Why cited: Amara-kośa lexical proof that 'sahasā' means 'unanticipated / unreasoned', supporting Jīva's gloss that trāsa lacks pūrvāpara-vicāra.

Putting it together: that same trembling of the heart, if it comes suddenly and shakes the limbs, is called trāsa. Jīva restates the definition as a plain if-then.

And precisely because fear is reasoned while trāsa is sudden, fear stands apart from trāsa — that is the sense of the verse's second line.
