aham — I; syamantakam hṛtvā — having stolen the Syamantaka jewel; ghora-āsyam antakam gataḥ — have gone to terrible-mouthed Death.
Having stolen the Syamantaka jewel, I have as good as gone to Death, whose mouth is terrible. Akrūra speaks in his own despairing voice.
An udāharaṇa (illustrative verse) of cintā, 'anxiety' — a vyabhicāri-bhāva of the fourth laharī (2.4). Jīva names it akrūra-cintā, Akrūra's anxious brooding after keeping the Syamantaka jewel, spoken in his own voice.

vaitaraṇīyam anu kṣiptaḥ — cast down toward the Vaitaraṇī; kāṁ vā taraṇīṁ karavai — which boat, then, am I to make?
Flung toward the Vaitaraṇī, the dread river of hell, what boat can I possibly make to cross it? The despair is sharpened by the sound-echo: taraṇī, a boat, inside vaitaraṇī, the very river he must cross — and he has none.
Śleṣa on sound: taraṇī (boat) sits inside Vaitaraṇī (the hell-river). A boat is exactly what the Vaitaraṇī demands, and Akrūra has none — the wordplay carries the anxiety.

syamantakam aham iti — the verse cited by its opening; akrūra-cintā — voices Akrūra's anxiety.
The verse 'syamantakam aham…' is Akrūra's cintā — his anxious dread. Jīva names both the speaker, Akrūra, and the mood, anxiety, which fixes what this udāharaṇa illustrates.

kāṁ vā ? ity atra tu "kiṁ vā" iti pāṭhaḥ sabhyaḥ ||
kāṁ vā ? ity atrahere, at the words 'kāṁ vā?''
tuhowever marks the shift to a textual note'तु' शब्द (पाठ-टिप्पणी की ओर मोड़ता है)
atra tu — but here; kāṁ vā iti — for 'kāṁ vā?'; kiṁ vā iti pāṭhaḥ — the reading 'kiṁ vā'; sabhyaḥ — is the polished reading.
At the phrase 'kāṁ vā?', Jīva records a variant reading. He judges 'kiṁ vā' to be the more refined (sabhya) one. He notes the variant without erasing the received text.
A pāṭhāntara (variant-reading) note: 'kāṁ vā' vs 'kiṁ vā'. Jīva calls 'kiṁ vā' sabhya, 'fit for the learned assembly'; Viśvanātha below carries both, writing kāṁ (kiṁ).

ghoram āsyaṁ mukhaṁ yasya tam antakaṁ gataḥ |
ghoram āsyaṁ mukhaṁ yasyahe whose mouth (āsya, i.e. mukha) is terrible bahuvrīhi unpacking ghora-āsya; āsyaṁ glossed by mukhaṁजिसका मुख (आस्य = मुख) भयंकर है — बहुव्रीहि

gataḥ[to whom I have] goneगया / पहुँचा
ghoram āsyaṁ mukhaṁ yasya — the one whose mouth is terrible; tam antakaṁ gataḥ — I have gone to that Antaka, Death.
Viśvanātha unpacks 'ghorāsyam antakam' as a bahuvrīhi. Antaka is Death; 'ghorāsya' means the one whose mouth (āsya, that is, mukha) is terrible. So the sense is: I have gone to terrible-mouthed Death.

kṣiptaḥ ahamI have been flungमैं फेंका गया हूँ
ata eva — for that very reason; vaitaraṇyāṁ kṣiptaḥ aham — I have been flung into the Vaitaraṇī.
And so, Viśvanātha continues, I have been cast into the Vaitaraṇī, the river crossed after death. His 'for that very reason' ties this to having just gone to Death.
Viśvanātha reads the river directly as a locative, vaitaraṇyāṁ ('into the Vaitaraṇī'), where the mūla has vaitaraṇīyam anu ('toward the Vaitaraṇī').

kāṁ (kiṁ) vā taraṇīṁ — what boat, then; karavai — am I to make?
What boat can I possibly make now? Viśvanātha restates the mūla's cry, keeping both readings, kāṁ and kiṁ. The pathos is plain: at the Vaitaraṇī one needs a boat, and Akrūra has none.
Śleṣa foregrounded: taraṇī (boat) echoes inside Vaitaraṇī (the hell-river of the line above) — the very crossing he dreads names the thing he lacks.
