Now the horn. It is made of buffalo-horn, and both its ends, the front and the back, are capped with gold.
First half of the lakṣaṇa (defining description) of the śṛṅga, one of the uddīpana-vibhāvas — the objects and gear of Kṛṣṇa that kindle the devotional mood. The illustrative verse (udāharaṇa) follows at L3-L6.

Its middle section flashes with a mesh of set jewels. This horn is known by the name Mandra-ghoṣa, 'the deep-sounding one.'

tārāvalīTārāvalī — a gopī (Viśvanātha: a yūtheśvarī, a leader of a band of gopīs); the name also puns on tārā-avalī, 'a row of stars'तारावली (नायिका / गोपियों की स्वामिनी)
veṇu-bhujaṅgamenaby the snake that is the flute — the flute cast as a serpentवेणुरूपी सर्प के द्वारा
tārāvalī — Tārāvalī; veṇu-bhujaṅgamena — by the snake that is [Kṛṣṇa's] flute
Tārāvalī is a gopī — Viśvanātha names her a yūtheśvarī, a leader of a group of gopīs. The verse casts Kṛṣṇa's flute as a snake.
The udāharaṇa (illustrative verse) for the śṛṅga defined at L1-L2. It runs on a sustained śleṣa (double meaning): the flute is a snake, its sound the snake's poison, and the horn's note is milk — with the folk belief (spelled out by Viśvanātha) that milk drunk after a snakebite doubles the venom.

tārāvalīlā-garalena daṣṭā |
tārāvalīlā-garalenaby the venom that is the effortless ease (avalīlā) of its high, ringing pitch (tāra) resolve as tāra-avalīlā-garala; Jīva and Viśvanātha both gloss avalīlā as alpa-prayatna, 'made with little effort' — the snake's fang is the flute's carelessly-brilliant high noteतार-अवलीला (उच्च स्वर का अल्प प्रयत्न) रूपी विष के द्वारा
daṣṭāshe is bittenडसी गई
tārāvalīlā-garalena — by the venom that is the effortless ease of its high pitch; daṣṭā — [Tārāvalī is] bitten
The flute-snake bites her with its venom — and that venom is the loud, high note the flute throws off so effortlessly. She is bitten by the sound itself.

Then she drinks the note of the horn as if it were milk — the horn-sound is the milk here.

viṣāṇi — the poisons [= her poison-like feelings]; kāmam — abundantly; dvi-guṇī-cakāra — she made twofold
Drinking that milk did not cure the venom — it doubled it. She doubled her 'poisons' many times over: her love-feelings, sharp as poison, only intensified. The pun turns on a folk rule Viśvanātha states — milk taken after a snakebite increases the poison.

idam ca — and this [word 'gavala']; upalakṣaṇam — is an upalakṣaṇa; kṛṣṇa-sāra-ādi-śṛṅgāṇām — of the horns of the kṛṣṇa-sāra antelope and so on
And Jīva adds: 'gavala' here is only an upalakṣaṇa. It names buffalo-horn, but it stands in for horns of other animals too, like the kṛṣṇa-sāra antelope. The horn need not be buffalo alone.
upalakṣaṇa vs viśeṣaṇa: a viśeṣaṇa states part of what the thing is (a 'blue lotus' — blue belongs to the lotus); an upalakṣaṇa merely helps you locate it and is not exclusive ('the house with the crow on it' — the crow only points, and the house stays a house once the crow flies off). Here 'buffalo-horn' works the second way: it identifies the item without ruling out antelope-horn and the rest.

Jīva unpacks the metaphor. Her name is Tārāvalī. 'Tāra' is the loud, high note; its 'avalīlā' is the ease with which it is produced, made with hardly any effort. That effortless high note is the venom, and the flute is the snake whose venom it is. The horn's sound, in turn, is the milk. Jīva calls the whole construction a rūpaka — a metaphor that identifies one thing outright with another.
rūpaka: a figure that does not merely compare (X is like Y) but identifies (X is Y) — here flute = snake, high note = venom, horn-sound = milk.

Jīva closes the logic. At first the horn's milk-sound is welcome, because one expects milk to quiet the flute-venom. But afterward it does the opposite: kept in the venom's company, the milk only feeds it. So the verse's word 'viṣāṇi,' the poisons, means her feelings that are the very equal of poison — love-feelings that sting like venom and only doubled when the horn sounded.

Viśvanātha runs the same picture and names its punchline. Tārāvalī is a yūtheśvarī, a gopī group-leader. The flute is a snake; its venom is the loud note it throws off so easily; she is bitten by it. Right after, she drinks the horn's sound as milk. That doubles her poisons — because, as everyone knows, milk taken after a snakebite only strengthens the venom. So the horn-note, far from curing her, redoubles her poison-like love.
