A woman burning in separation cries out against a fever. First mark: it scorches — hotter, she says, than the puṭa-pāka, the sealed-crucible firing that purifies gold. Second mark: it throws her whole being into turmoil, worse than a heap of poison could.
An udāharaṇa of vyādhi, 'love-sickness,' the vyabhicāri-bhāva under discussion here ('atha vyādhiḥ'); the verse is quoted from the same source (tatraiva), 3.28. The speaker is a woman pining in separation from Kṛṣṇa; Viśvanātha (below) names her as Śrī Rādhā speaking to Lalitā. puṭa-pāka is an Ayurvedic/alchemical process: a substance is sealed inside a clay vessel and heated fiercely — a byword for extreme heat.

Third mark: it is harder to endure than Indra's own thunderbolt (dambholi). Fourth mark: it cuts sharper than a barbed dart sunk deep in the heart. Each comparison reaches for the worst pain she can name, then says the fever is worse.

marmāṇy adya bhinatti gokula-pater viśleṣa-janmā jvaraḥ ||
marmāṇy adya bhinattitoday it splits apart my vital points marmāṇi accusative pl, the tender life-spots; adya 'today, now'; bhinatti 'pierces, cleaves'मर्मस्थानों को आज (इसी समय) बेध रहा है — मर्म वे कोमल जीवन-स्थल हैं जहाँ आघात प्राणघातक होता है
gokula-paterof the Lord of Gokula — Kṛṣṇa, from whom she is parted genitive, going with viśleṣa: the separation is from himगोकुलपति (कृष्ण) का — यह 'विश्लेष' के साथ जुड़ता है, अर्थात् उन्हीं से वियोग
viśleṣa-janmāa fever born of that separation bahuvrīhi qualifying jvaraḥ — viśleṣa 'being parted' + janman 'birth, origin'; this names the true causeवियोग से उत्पन्न — यही ज्वर का सच्चा कारण बताता है
jvaraḥthe fever — the subject all six marks describe nominative sg, the grammatical subject held to the end of the verseज्वर (यही वाक्य का कर्ता है, जो पद्य के अन्त तक टिका रहता है)
gokula-pater viśleṣa-janmā ayaṁ jvaraḥ — this fever, born of separation from the Lord of Gokula; adya mama marmāṇi bhinatti — today splits apart my vital points.
Now the turn that makes it love-sickness, not illness: this fever is 'born of separation from the Lord of Gokula' — from Kṛṣṇa. That is its real cause. And today it splits her marmans, her vital points — the idiom for pain that reaches where life itself is held. All the fire, poison, thunderbolt, and cholera were only ways to measure the pain of being parted from him.
marman: in Ayurveda the vital junctures of the body where a wound is mortal; 'splitting the marmans' is the standard idiom for pain that strikes at the seat of life.
