mahiṣṭhaḥ mānaḥ — the greatest māna; śreyasā api upāyena — even by the most excellent device; duḥsādhyaḥ — hard to pacify; kṛcchra-sādhyaḥ syāt — that is, could be won over only with great difficulty.
Jīva restates the top grade. The greatest māna is duḥsādhya — hard to pacify — even by the finest means. He unpacks duḥsādhya as kṛcchra-sādhya: it yields only with great trouble.
Jīva's phrasing has śreyasā ('a superior means') where the verse reads preyasā ('the beloved'); the two readings are left as they stand, not merged.

hetor īrṣyāyāḥ ||
hetoḥthe verse's word 'of its cause' the lemma Viśvanātha is pinning down, from L1'कारण' शब्द का (L1 का वह लेम्मा जिसे विश्वनाथ स्पष्ट कर रहे हैं)
īrṣyāyāḥmeans: of jealousy genitive in apposition — identifies the cause as īrṣyāअर्थात् ईर्ष्या का (अपादान-रूप षष्ठी, कारण की पहचान)
hetoḥ — the verse's 'of its cause'; īrṣyāyāḥ — [means] of jealousy.
Viśvanātha names the 'cause' in the verse: it is īrṣyā, jealousy. So the grade of māna follows the grade of the jealousy behind it.

bhujaṅgaḥ — the word bhujaṅga; kāmukaḥ — means a lover, a paramour.
A lexical gloss: bhujaṅga means kāmuka, a paramour. The preceding '(129)' is the concordance number for the verse.
bhujaṅga literally is 'snake'; it is carried over for a sneaking, prowling lover — the etymological force behind Viśvanātha's gloss kāmuka.
