The thing is made of a bright-red tender leaf. Pressed into it are nail-marks — little crescents and other shapes.
The lakṣaṇa (defining verse) of the nirakṣara, the 'letterless' love-letter — one of the kinds of kāma-lekhya. The lead-in tatra nirakṣaraḥ marks it out: 'among these [love-letters], the letterless one—'.

An udāharaṇa of the nirakṣara defined in the previous verse (15.65). Viśvanātha sets the scene: the girlfriend Viśākhā hands Kṛṣṇa this letterless love-letter; he presses it to his heart, and a moment later speaks this verse to Subala. So the 'I' of the verse is Kṛṣṇa.

iha — here; madanārdha-candra-bhāvaṁ — the nature of Madana's crescent-arrow; dadhad — taking on.
Here it takes on the very nature of Madana's crescent-arrow. The nail-drawn half-moon has become Kāma's ardha-candra shaft.
The poem pivots on ardha-candra: the same words name the crescent scratched on the leaf and the ardha-candra, one of Kāmadeva's named arrows (a crescent-headed shaft). So an innocent leaf-drawing is at once Kāma's loaded arrow. Viśvanātha makes this explicit in the next sentence — 'Madana's ardha-candra' means the arrow of that name, its likeness in property to that arrow (śara-sādharmya).

tan-nāmameans the [arrow] that bears that very name tat 'that' + nāma 'named': the arrow actually called ardha-candraउसी नाम का — अर्थात् 'अर्धचन्द्र' नामधारी बाण
śara-sādharmyamthat is, its sharing the properties of that arrow sādharmya = likeness in properties — it acts as the shaft actsउस बाण के साथ धर्म-साम्य — वह उसी शर की भाँति काम करता है
ity arthaḥsuch is the meaningऐसा अर्थ है
madanasyārdha-candra-bhāvaṁ — 'the nature of Madana's crescent'; tan-nāma — means the arrow of that name; śara-sādharmyam — its likeness in property to that arrow; ity arthaḥ — this is the meaning.
Viśvanātha unpacks 'the nature of Madana's ardha-candra': ardha-candra is the actual name of one of Kāma's arrows. So the nail-drawn crescent shares the property of that arrow — it wounds just as the shaft it is named for does.
