vighaṭiteare rolled back, disarranged dual — points to the two eyes (dṛṣṭī); the poet's dṛṣṭiḥ carries the dual senseजब दोनों (नेत्र) विघटित/निश्चल हो गए (द्विवचन — दोनों नेत्रों की ओर संकेत)
dṛṣṭiḥ[her] eyes, [her] gazeदृष्टि (नेत्र-दृष्टि)
snuṣāyāḥof my daughter-in-law genitive; snuṣā = 'daughter-in-law' — so the speaker is Rādhā's mother-in-lawमेरी पुत्रवधू की (snuṣā = पुत्रवधू; अतः बोलने वाली राधा की सास है)
kathaṁhow [has this come about]?यह कैसे (हो गई)
snuṣāyāḥ — my daughter-in-law's; nāsā-śvāsa-parāṅ-mukhī — nose has turned away from breath; dṛṣṭiḥ — [her] eyes; vighaṭite — are rolled back; kathaṁ — how?

'My daughter-in-law's nose has stopped breathing; her eyes have rolled back — how did this happen?' These are the alarmed words of Jaṭilā, Rādhā's mother-in-law.
मेरी पुत्रवधू की नासिका से श्वास हट गया है, और दोनों नेत्रों के विघटित (निश्चल) हो जाने पर उसकी दृष्टि — यह कैसे हो गई!
The udāharaṇa of moha, quoted from Vidagdha-mādhava 2.3, illustrating the moha just defined in the previous line. The frame is Viśākhā reporting the scene to Kṛṣṇa; inside it she quotes Jaṭilā, Rādhā's mother-in-law, who takes the swoon for spirit-possession.

'Alas, fie! Put the black sesame seeds into my hand, so I can do the exorcism-rite [to drive the spirit out of her].' Jaṭilā means kṛṣṇa-tila as nothing but 'black sesame' — but the two syllables kṛṣ-ṇa are about to do their real work.
हाय, धिक्कार है! काले तिल मेरे हाथ में दे दो, जिससे मैं अपमार्जन (झाड़-फूँक) कर सकूँ।

karṇayoḥ parisaraṁthe neighborhood of [Rādhā's] two earsदोनों कानों के समीप
kṛṣṇeti varṇa-dvayenamely the two syllables 'kṛṣ-ṇa' varṇa-dvaya = 'a pair of syllables'; locative absolute, the thing that climbs'कृष्ण' इन दो अक्षरों के (सति-सप्तमी — जो चढ़ते हैं वही)
iti — with these words [of Jaṭilā]; kṛṣṇeti varṇa-dvaye — the two syllables 'kṛṣṇa'; karṇayoḥ parisaraṁ — to the neighborhood of [her] two ears; ārohati — as they climb up.
iti — इस प्रकार; kṛṣṇeti varṇa-dvaye — 'कृष्ण' इन दो अक्षरों के; karṇayoḥ parisaraṁ — दोनों कानों के समीप; ārohati — चढ़ आने पर
As Jaṭilā speaks, the two syllables kṛṣ-ṇa — hidden inside her word for black sesame — climb to the edge of Rādhā's ears. She meant only 'black'; but the Name Kṛṣṇa has reached the beloved's ear.
इस प्रकार (जटिला के) कहने पर, जब 'कृष्ण' ये दो अक्षर दोनों कानों के समीप चढ़ आए...
This is the śleṣa the whole udāharaṇa pivots on: kṛṣṇa in kṛṣṇa-tila means only 'black', yet the identical syllables are the Name Kṛṣṇa. Jaṭilā, busy suspecting a ghost, has no idea; it is the bare sound of the Name that stirs Rādhā.
यही वह श्लेष है जिस पर सारा उदाहरण घूमता है: 'कृष्ण-तिल' में 'कृष्ण' का अर्थ केवल 'काला' है, फिर भी वे ही अक्षर कृष्ण नाम हैं। जटिला भूत की शंका में लगी है, उसे कुछ पता नहीं; नाम की केवल ध्वनि ही राधा को उद्वेलित कर देती है।

Jīva unpacks sūtritavatī: it means she [the girlfriend] let me know — briefly, in a few private words. 'Threaded' here is not composing a text but quietly whispering the point that Kṛṣṇa is the cause.
'सूत्रित कर दिया' का अर्थ है — संक्षिप्त वचन से, एकान्त में, मेरे प्रति जता दिया।
