An udāharaṇa of prauḍha prema — the 'mature, bold' grade of Kṛṣṇa's love flagged just above by the lead-in 'tatra prauḍhaḥ' ('there, the mature one'). The whole verse is Kṛṣṇa's speech: he sends his friend with a message for his waiting beloved, Kamalā. Viśvanātha (s5) names the two things it illustrates — prema, and prema's prauḍhatva.

cikitsan frames the demon-fight as a cure: the demon is the disease (śiraḥ-śūla, a piercing headache), Kṛṣṇa the doctor. The conceit is why he speaks of 'reaching your couch' in the next line — a physician's playful boast of curing fast and coming straight to her.
