sādharmyaṁ madhuripu-viprayoga-bhājāṁ
sādharmyaṁthe shared likeness — the very traits [she has in common with them] acc., object of tanoti in L2; governs the gen. that followsसमानता को — जो अगली पंक्ति की क्रिया 'तनोति' का कर्म है; और आगे आने वाली षष्ठी को अपने अधीन रखता है (जिनके साथ की समानता)
madhuripu-viprayoga-bhājāṁwith those who undergo separation from Madhu-ripu (Kṛṣṇa, the foe of the demon Madhu) gen. pl. governed by sādharmya, 'likeness with'; bhāj = 'one who partakes of / undergoes'मधुरिपु (मधु दैत्य के शत्रु कृष्ण) के वियोग को भोगने वालों की — षष्ठी 'समानता' के अधीन; भाज् अर्थात् जो किसी दशा को भोगता या सहता है
sādharmyaṁ — the shared likeness; madhuripu-viprayoga-bhājāṁ — [the likeness] with those who suffer separation from Kṛṣṇa.
The likeness she bears is the likeness of people undergoing separation from Kṛṣṇa — Madhu-ripu, the slayer of the demon Madhu. The sentence finishes in the next line.
An udāharaṇa of anumāna — 'liṅgād ūho'numānatā', an inference drawn from a tell-tale sign — the nāṭyālaṅkāra defined in kārikā 86; quoted from Candrāvalī 6.23 (act 6), where a watcher reads the marks on a woman's wasted body and infers her hidden longing for Kṛṣṇa.

iyam tanvaṅgī — this slender-limbed woman; aṅgakaiḥ — through her limbs; muhuḥ — again and again; [sādharmyaṁ] tanoti — spreads out [that shared likeness].
This slender girl, again and again, spreads that likeness out through her very limbs. Read the sign: her wasted body carries every mark a Kṛṣṇa-separated lover shows, so one infers that she too is pining for him. That reading of an inner state off a bodily sign is exactly the anumāna the kārikā just defined.

śriyamthe splendor, the beauty acc., object of prathayitum in L4शोभा को, श्री को — जो अगली पंक्ति की क्रिया 'प्रथयितुम्' का कर्म है
mādhavīṁspring-like, of the Mādhava season (qualifying that splendor)माधवी — दो अर्थ एक साथ: वासन्ती (माधव मास अर्थात् वसन्त की सी) तथा माधव (कृष्ण) से सम्बन्ध रखने वाली शोभा
kimcan it really be that…? — a note of wonder interrogative opening the rhetorical question completed in L4क्या — यह प्रश्नवाचक शब्द है, जो अगली पंक्ति में पूरे होने वाले प्रश्न को आरम्भ करता है
enāṁthis woman — the one they make show it forth acc., the one caused to display by ārtayaḥ in L4इसको — जिसे अगली पंक्ति की 'आर्तियाँ' प्रकट करने में समर्थ हैं
kim — can it be that…; enāṁ — [make] this woman; ākṛtyā — by her figure; mādhavīṁ śriyam api — even a spring-like splendor…
And here is the wonder: by her very figure she shows forth even a spring-like splendor (mādhavī śrī). The question that carries this runs on into the next line.
mādhavī plays on two senses at once: 'of the spring / vernal' (mādhava = the spring month) and 'belonging to Mādhava' (Kṛṣṇa) — so her form takes on a beauty that is at once spring-fresh and Kṛṣṇa-like, echoing the sādharmya of L1: in longing she comes to resemble her beloved.

Can it really be that her pangs of separation — even in this wretched, wasted state — have the power to make her body bloom with that splendor? The marvel is that her very suffering only makes her more radiant.

This slender-limbed woman, again and again, spreads through her very limbs the likeness of those who suffer separation from Kṛṣṇa, the slayer of the demon Madhu — every wasting mark of a Kṛṣṇa-parted lover shows on her body. Can it really be that her very pangs of longing, even in this wretched, wasted state, have the power to bring forth from her figure a splendor at once spring-fresh and Mādhava-like? The marvel is that the suffering which should only diminish her instead makes her bloom.
An udāharaṇa of the nāṭyālaṅkāra anumāna ('liṅgād ūho’numānatā', inference from a tell-tale sign), defined in kārikā 86; quoted from Candrāvalī 6.23, where a watcher reads the marks on a woman's wasted body and infers her hidden longing for Kṛṣṇa.
